The organic mastic under the tile was allowing the tile to bend under the concentrated weight. If I had used cement-based thinset instead of mastic, the tile wouldn’t have cracked.

Much to my surprise, I found a concrete mortar bed under the tile. I’m assuming I must remove the mortar and add more wood sub-flooring on top of the existing plywood subfloor.
